How to place a request:

(1) Place request through website

After registering as a member at our website, a request can be placed by navigating to "Inquiries" - "Free rental desired."
You can register for a website membership here.

(2) Place request by fax

You can also place a request by filling out the necessary information in the Testing Unit Free Rental Request Form and faxing it to us.
